New Patient Questionnaire and Intake Form Builder for Easier Data Collection by Healthcare Providers

BodySite launches a patient questionnaire and intake form builder to make collecting patient data through forms easier for the patient and more productive for the provider – NOW WITH TEMPLATES!

Easily send secure electronic intake forms and questionnaires to your clients and patients and receive them back prior to encounters. This is a game changer for onboarding patients and progress follow ups between visits.

It’s often difficult for patients to relay their actual symptoms and current conditions to their health care provider during office visits. In fact, most patients are either dishonest or just aren’t providing enough information during their visit.

In fact, the JAMA Network Open medical journal cites that in a study conducted “1630 of 2011 participants (81.1%)” reported non disclosure to their health care providers. The reasons range from embarrassment, confusion, etc. According to Televox’s Healthy World Report “34 percent of American adults are more honest about their medical needs through automatic calls, email, or text messaging, than they are in a face to face conversation with a doctor.”

Worse yet, the process of collecting information from patients is time consuming, inefficient and contributes to most of the patient dissatisfaction at encounters. Patients simply don’t want to sit around and fill out forms with pen and paper. It’s annoying, often not secure and creates more staff time transposing that information into your electronic systems.

Despite this, good doctors understand the extreme importance of providers gaining as much insight as possible to their patients’ symptoms and current condition. And surveys and questionnaires foster the unique opportunity for patients to share information openly and privately with their healthcare provider.

So how do you bridge the gap between gathering sufficient patient information and the barriers involved with easy and comprehensive patient disclosure? The answer… Administer assessments virtually, with electronic forms.

Get rid of paper intake forms. Your patients will be glad you did.

BodySite’s Intake Forms and Questionnaire Builder – Your Virtual Solution to Facilitate Patient Reporting

So if patients feel more comfortable providing information up in a virtual format, there’s no better place to correspond with them than the BodySite digital remote care platform.

The BodySite platform already gives providers the ability to deliver remote care guidance to patients and monitor everything that their patients are doing in realtime. This includes gathering nutritional information, biometrics and activity data, scheduling telemedicine encounters and deploying care information to patients including diet, exercise and mindfulness advice in turnkey plans.

Combine these features with health intake forms and questionnaires to get the full scope of your patients’ health at the beginning of the relationship to streamline onboarding. And better yet, use the questionnaire engine to continue gathering patient information throughout the patient life cycle to make care adjustments as needed.
