How to Use BodySite’s Questionnaire Builder
(1) Log on to your BodySite portal
(2) Click the “Libraries” drop down and head to “Forms/Surveys”
(3) Click the “Create New Form/Survey” button and name your survey
(4) Click the pencil icon to edit the Survey Description
(5) Click Design the survey to begin creating questions for your patients. They can be in the form of yes/no, multiple choice, text entry and more
(6) Create and edit the form/survey sections if your form/survey requires it
(7) Once completed, head back to settings and click “Form/Survey is in Draft Mode” to publish the survey to use with patients
(8) To deploy a survey to a patient, head to the “Patients” tab at the top of the page
(9) Open any patient record to assign a survey (surveys/questionnaires can also be used with new patients upon enrollment)
(10) Click “Manage Forms/Surveys” near the bottom, middle section of the page.
(11) From this modal, you can preview and assign your survey to your patient. You’ll be able to assign the survey you created or any of BodySite’s stock surveys (if you don’t see your new form/survey, go back to #7 on this list to ensure your survey is published).
(12) Click “Assign Survey” to deploy the survey to your patient
(13) Once you’ve completed those steps, you’re all set. Your patient will receive a confirmation email about the assigned survey via email and will be prompted to fill out the survey on the web portal.
(14) You will receive their results once they’ve completed it and can optionally share the results with your patient.
